The Student Association at Durham College, UOIT and Trent in Oshawa (SA) is pleased to offer an even better health plan to full-time students this year!
Highlights of these New Benefits include:
• Vision Care
• Orthopedic Supplies
• Prosthetic Appliances
• Clinical Psychologist or Speech Therapist (if recommended by doctor)
• Naturopath or Chiropractor
• Physiotherapist (if recommended by doctor)
• Massage Therapy (if recommended by a doctor)
• Medical Supplies
As in the past, the health plan also covers 80% of the cost of most prescription drugs, including all vaccines (immunizations) up to a maximum of $3000 for one policy year, September 1-August 31.
New This Year!
The Campus Health Centre is expanding to a new location on-site in the Recreation and Wellness Centre and is pleased to announce that a pharmacy, Lovell Drugs, will be housed within this space. This is great news for students who will now be able to access their prescription medicines on campus. The Pharmacy has committed to capping the dispensing fee at $7 for oral contraceptives, injectible contraceptives, the nuvring and birth control patch ONLY. All other prescriptions will be charged the normal dispensing fee.
Dental Benefits
Students are able to access the following dental benefits:
• 100% of one examination and consultation, necessary x-rays and diagnostic services during each policy year
• 100% of one cleaning and one unit of polishing
• 75% of the cost of fillings and space maintainers
• 75% of the cost of two wisdom teeth extractions

Opt Out and In Process
Opt Out
Students may decline coverage for either or both plans (health/dental) by going to www.aclassociates.com/sadcuoit.htm and completing the online opt out form. You will be asked to provide the name of the insurance company, policy number and name of the person who has the plan.
Opt in
Students on the health/dental plan may opt in their family members on line with a VISA card by going to www.aclassociates.com/sadcuoit.htm. Students may also opt in manually with a certified cheque or money order in the SA Office. Family members must be opted in by the deadlines below (same as the opt out deadlines).
Opt out and in deadlines are as follows:
September-start students: Begins September 1, 2007
Deadline September 21, 2007
January-start students: Begins January 1, 2008
Deadline January 22, 2008
February-start students: Begins February 1, 2008
Deadline February 22, 2008
*May-start students: Begins May 1, 2008
Deadline May 21, 2008
*May 16-start students: Begins May 8, 2008
(14 week term) UOIT only Deadline May 28, 2008
Students are encouraged to opt out by the deadlines shown as there will be NO exceptions made if the date is missed. *May-start students must complete a paper opt out through the SA office and provide proof of coverage.
REFUND CHEQUES MUST BE PICKED UP WITHIN 6 MONTHS OF OPTING OUT! STALE-DATED CHEQUES WILL NOT BE REISSUED. CHEQUES ARE ISSUED IN THE NAME OF THE STUDENT AND CURRENT STUDENT ID MUST BE PRESENTED IN ORDER TO RECEIVE A CHEQUE.
